Before your second opinion appointment, it is important you provide us with all relevant clinical information regarding your child's initial diagnosis, including:

Your child's initial diagnosis and their physician's contact information
Written records reporting results from any current blood test
Written records reporting the results of blood testing done prior to your child's illness
Any other relevant written information you have from other specialists
For certain genetic blood diseases, this includes any results of blood testing on parents
